Telemedicine

 

Thank you for your interest in our telemedicine service

  • Please be aware in order to have a seamless video consultation you must have a strong internet connection.
    • Please avoid hotspots with only one or two bars
    • If you do not have internet please locate your nearest library with free wifi and a private booth
    • Avoid public internet because public internet performance is unreliable.
  • For Tele-medicine appointment please call us at 480.300.GOMD (4663) to schedule an appointment
  • If you already have an appointment please go to http://telemedicine.aveonhealth.com

 

Telemedicine on Phone or Tablet

  1. After you have already scheduled your appointment with us please install the “ContinuousCare Health App” from the iPhone-app store or Android-Play Store.
  2. Search for the app called “ContinuousCare Health App”
  3. Download and install the app
  4. Once the app has installed tap on the app to open it
  5. Log in with the email address you used at the time you called in to set up the appointment
    1. If you do not have a password click forgot password inside the app to setup a password (please check your spam folder if you do not see the password reset email in your inbox)
  6. Once you have logged in within the APP then tap on consultations and then tap video consultation
  7. Tap on pay to be redirected to PayPal to process your payment
  8. Once payment has been processed tap on start session five minutes before your appointment time to start your consultation

Telemedicine on Windows or Mac

  1. After you have scheduled your appointment with us please visit http://telemedicine.aveonhealth.com
  2. Log in with the email address you used at the time you called in to set up the appointment
    1. If you do not have a password click forgot password inside the app to setup a password (please check your spam folder if you do not see the password reset email in your inbox)
    2. You may be asked to accept the user agreement please click accept to continue
  3. Once you have logged in click appointments located on the left column of the screen
    1. If you are unable to see appointments please click the three white lines on the upper left corner of the screen to show the menu
  4. Once you have clicked appointments click on pay now link to fill in the billing details and click on Proceed To Payment button
  5. You will be directed to PayPal to process your payment
  6. Once payment has been processed click on start session five minutes before your appointment time to start your consultation
